Paid Advertisement Ā· Imago Dei Direct Primary Care
What If Your Physician Had Time for You?
At Imago Dei Direct Primary Care in Zeeland, Dr. Daniel Swartz (āDr. Danā) has built his practice around a simple idea: you should know your physician, and your physician should know you.
Dr. Dan moved to West Michigan to be closer to his grandchildren and extended family. He brings more than 20 years of Family Medicine experience, including six years in direct primary care, to caring for patients from childhood through older adulthood.
The model is straightforward: members pay a monthly fee for unlimited, ongoing primary care. Imago Dei does not bill insurance, Medicare, or Medicaid for its services. That approach allows Dr. Dan to keep his practice smaller and make more room for conversation, questions, and getting to know the person in front of him.
Membership includes 30ā60 minute appointments, direct phone and email access to Dr. Dan, and telehealth when appropriate. The practice says appointments are almost always available the same or next day. Membership includes preventative care, illness and injury visits, chronic-condition management, and a wide range of clinic procedures.
The name Imago Dei means āthe image of God,ā reflecting Dr. Danās Christian belief in every personās God-given worth and dignity. The practice welcomes all patients.
Straightforward membership pricing
$87 per month for an individual
$250 per month for an eligible family of five
An initial registration fee is $87 per person, with a $174 family maximum.
The practice defines family as married or single adults plus biological or adopted children under 23 and adult dependents unable to live alone. Adults 23 and older, and independent or married adults over 17, need their own memberships.
Most clinic procedures are included in membership; a few procedures carry additional charges. Laboratory and pathology testing carry additional fees. The practice advertises discounts of up to 90% on laboratory testing, with discounted pathology pricing also available. Some supplies also cost extra. DPC is not insurance and does not replace coverage for hospitalizations, specialists, or other outside care.
While Dr. Dan loves caring for families, Imago Dei also offers small businesses and nonprofits a way to provide employees with a primary care benefit. The membership is designed to support employee health through accessible, ongoing care. Transparent business pricing is available on the practice website.
Curious whether DPC fits your family, business or nonprofit? Call 616-407-4968 or email [email protected] to arrange a no-cost, no-obligation, no pressure, meet-and-greet with Dr. Dan. The office is located at 8516 HomesteadĀ Drive, Suite 102, Zeeland. Membership spots are limited.
